Connie is an experienced home care nurse and administrator. With over 30 years of home care experience, she has worked in clinical, leadership and regulatory roles. Her background spans assisted living, private duty, and Medicare-certified agencies. Her primary areas of focus are home care development, systems management, program development and evaluation, leadership, quality improvement, orientation and education, funding, and regulatory compliance. She is also a part-time surveyor for the Accreditation Commission for Health Care (ACHC) and serves on the faculty of Bethel University. |
Nazneen Khatoon
Nazneen has been in management and administration for over 20 years. For the past 18 years, she has been actively involved in the home care and assisted living industry. She is knowledgeable in business applications, billing, payroll and HR/personnel functions. Her areas of expertise are identifying issues, problem solving and teaching and implementing successful action plans. She is professional, competent and outcomes-focused. |
Geri Wagner, RN, BSN, MMA
Geri has worked for over 35 years in home care, hospice, assisted living and residential hospice. Her areas of focus include policies and procedures, staff training and development, oversight for compliance with state, federal and accreditation requirements, quality improvement, strategic planning and fiscal and clinical management. Geri has served on numerous Minnesota Home Care committees and has testified before the state legislature on issues related to home care. |
Pat Chamberlain, BAN, RN, PHN, COS-C
Pat is an experienced homecare nurse and educator. She has 40 years of nursing experience with a background in cardiac nursing, home infusion, homecare, and clinical education in homecare. Pat’s primary areas of focus are orientation, education, and home infusion, Medicare guidelines, and OASIS documentation. She is certified in OASIS C. She is also a clinical Educator for Medicare-certified home health agencies. |
Cathy Griffin, B.S.
Cathy has over 40 years of experience in health and human services including more than 30 years specifically developing programs and services for people who need long-term services and supports. She has been employed in a variety of settings including county and state human service agencies, a non-profit health system, a for-profit home health agency, the state education agency, the state department of health and her own consulting and event planning organization. Cathy holds a Bachelor of Science Degree in Speech Communication from the College of St. Catherine, is a member of the National Alliance for Medicaid in Education of which she is a founding member and former President, and volunteered with the MN Twins for 25 years to raise funds for their Community Fund. |
